Mohan is the granddaughter of paravur T. K. Narayana Pillai, the first Chief Minister of the erstwhile Cochin State after Independence. Her first song was Kalai Paniyil from the Tamil Film Gayathri (1977), starring Sridevi Rajinikanth, and music composed by Ilayaraja. Her first Tamil hit film song was Puthu Vellai Mazhai from Mani Ratnam's 1992 film Roja, composed by A. R. Rahman. She has had other hit songs from films including Gentleman (1993), Indian (1996) and Minsaara Kanavu (1997).

She has sang in Malayalam, Telugu, Tamil, Kannada, Hindi, and Baduga languages. Her Hindi songs in films like ‘Pukaar’ and ‘Taal’ have topped the charts. She has sang more than 5000 songs.
